Consent to process personal data when submitting an enquiry

Controller and purpose

Freely, of my own will and in my own interest, I authorise the controller — OOO “MEDOVIK ALTAYA” (ООО «МЕДОВИК АЛТАЯ»), INN 2222797246, KPP 224601001, OGRN 1112223007632, registered address: 5 Budennogo St., Kalmanka village, Altai Krai, 659040, Russia — to review my enquiry about products, a price list or cooperation, prepare a response, contact me using the details I provide, and conduct negotiations related to that enquiry.

Data and processing operations

This consent covers the personal data I provide in the enquiry form and in further communications about the same matter: name, telephone number, email address, the enquiry and responses. If I provide my organisation or job title, they may be processed only as necessary to address the matter.

The permitted operations are collection, recording, organisation, accumulation, storage, updating, retrieval and use for this purpose, as well as blocking, deletion and destruction. Processing uses the website and work email systems; authorised staff of the controller handle the enquiry.

To confirm receipt of consent, the controller retains a record of the consent, the date and time it was given, and the version of the consent in effect at that time.

Limits

This form does not require passport copies, bank documents, health information or other data unrelated to the initial enquiry. I provide my own data. Processing other people’s data requires a lawful basis.

This consent does not cover advertising emails, advertising calls, publication of personal data or optional website technologies. Processing documents for a concluded contract and keeping mandatory records require separate lawful grounds. This consent is not blanket permission to transfer data to any third party or any country. Engaging processors and transferring data abroad require a separate assessment of the basis and compliance with the statutory procedure.

Duration, withdrawal and refusal

This consent lasts until the response and related negotiations are completed, I decline further negotiations, or I withdraw consent, whichever occurs first. It does not authorise storage for an indefinite future purpose. After the purpose is achieved or withdrawal of consent is received, the controller stops processing and destroys the data in accordance with the procedure and time limits established by Russian personal data legislation, unless there are other lawful grounds for continued processing.

I can withdraw consent or request information about processing at info@medovik-altay.ru or by writing to the controller’s registered address. I should provide the contact used for my enquiry and enough information to locate it; a passport copy is not required by default. If verification requires more information, the controller requests only what is necessary.

After withdrawal, processing may continue only to the extent supported by an independent lawful basis, including necessary evidence of consent and withdrawal; the scope and duration of that retention must be justified. Retaining these records does not in itself authorise continued negotiations or use of the data for advertising.

I give consent by making a separate, initially unticked choice beside the form and submitting my enquiry. This form cannot be submitted without that choice. I can contact the controller directly; the basis and necessary scope of processing that communication are determined separately.
